What Is Herniated Disc ?

Herniated discs are extremely common back injuries. Herniated disc disease is due to a change in the structure of the normal disc. Most of the time, disc disease comes as a result of aging and the degeneration that occurs within the disc. Occasionally, severe trauma can cause a normal disc to herniate. Herniated Discs: Herniated Discs a major Cause of Back Pain

Herniated disc – just the name brings shivers down my spine! More serious than a muscle strain it is one of the more common causes of back complaints. What is a herniated disc? How can it be treated? How can it be prevented? Discs are the cushion of material between your spinal. You have twenty three of these discs. Each one acts as a cushion between the vertebrae. Disc consists of a tough outer membrane and a soft inner layer. When too much pressure is placed on the discs some of the material can leak or bulge out and press against a nerve root. This is commonly called a pinched nerve. It’s this pinched nerve that causes the pain and discomfort.

Treatment: Although it sounds serious, many times the best treatment is rest, icing with an ice pack and taking a medicine like Tylenol. The good news is that only about 2% of people with this condition require surgery.

Prevention: Most back problems can be eliminated by using proper lifting techniques. Lift with your legs not your back and don’t lift more than you can comfortably handle. Degeneration of your discs is something that happens as we age. However by strengthening core muscles and preventing muscle imbalance most back injuries can be prevented.

Good Habits Help You Avoid Back Injuries At Work

Most back injuries on the job are due to a combination of factors. Some of them can be affected by changing your lifestyle and taking a look at:
• Your weight
• Your degree of fitness
• Your muscle conditioning
• Your flexibility

There are four factors in the workplace which are related to the increased risk of back injury:
• Force – if your job is physical and you have to lift or move heavy objects often, exerting too much force on your back may cause an injury.
• Repetition refers to the number of times that you must perform a certain movement. This can lead to muscle fatigue or injury, especially if your body is positioned awkwardly or you are stretching to the end of your range of motion.
• Posture is the position in which you spend the most time. For instance, sitting at a desk or bending over a computer all day will bring aches and pains. On the average, your body can usually tolerate being in one position for about 20 minutes before you feel compelled to move.
• Stress is increased by pressures from home and work, leading to muscle tension and tightness, which may develop into back pain or result in an injury.

Making Your Workplace Safer
Minimize Hazards
• Remove boxes, stools, and other obstacles from your area that might cause you to trip or fall.
• Look at the setup of your office or workspace. What can you do to modify it to reduce the strain of repetitive tasks and reduce physical demands? The goal is to decrease force and repetition while maintaining safe posture.

Pay Attention To Your Posture
• Poor posture places stress on your back. When you slouch, stand slumped or swaybacked, the back’s natural curves are exaggerated and this can lead to muscle fatigue and possible injury. Good posture relaxes your muscles and minimal effort is required to maintain your balance.
• If you sit at a desk, get a chair that supports your lower back.
• If your chair does not support the curve in the lower back, tuck in a small towel roll or pillow.
• Do not sit with a wallet or bulky item in a rear pocket pressing on your back as this can create pain and disrupt balance in your lower back.
• Adjust the height of your chair so your feet stay flat on the floor.
• If you work at a computer, make sure the chair and the monitor are positioned properly.
• If you use the phone, do not cradle it between your ear and shoulder as this definitely contributes to neck and shoulder pain.
• If you stand for long periods, rest one foot on a stool or small box periodically to relieve the strain on your back.
• Hold reading material at eye level when you are standing.
• Try to avoid bending forward when using the computer, handling desk work or completing handwork. This helps relieve strain on neck, shoulders and upper back.

Adopt Healthy Habits
• Change your position often and take a stretch break. The Mayo Clinic recommends a 30 second timeout every 15 minutes or so to stretch, relax, or move.
• Wear sensible shoes. Low heeled shoes with non-slip soles are preferable to high heels.
• Work on your coordination and balance. Do exercises to improve your balance and keep you steady on your feet.
• Regular walking is good exercise for the back and helps with your balance.
• Try to minimize the sources of stress on the job and at home.
• Develop coping mechanisms when you feel especially stressed.
• Perform deep breathing exercises, take a brief walk, or talk to a trusted friend.

Plan Your Moves
• If you are carrying something heavy, know exactly where you are going to put it and clear the area of obstacles and clutter.
• Avoid carrying heavy bags, briefcases, and purses whenever possible. Try to lighten the load that you carry on a daily basis.
• Avoid unnecessary twisting, bending, reaching.
• Lift properly by letting your legs do the work and keeping a heavy weight close to your body.
• If your back hurts, stop the activity and give the muscles a rest. Evaluate what you are doing to determine whether there is a better way you can complete this task without discomfort.
• Ask for help if a load is too heavy.

You can avoid back pain and injury by first understanding the causes and then making necessary changes in your work environment and your habits. To protect your back and maintain your well being, your focus should be on prevention.

Symptoms of a Herniated Disc

Common symptoms of a herniated disc include:

  • Electric Shock Pain:
    Pressure on the nerve can cause abnormal sensations, commonly experienced as electric shock pains. When the compression occurs in the cervical (neck) region, the shocks go down your arms, when the compression is in the lumbar (low back) region, the shocks go down your legs.
  • Tingling & Numbness:
    Patients often have abnormal sensations such as tingling, numbness, or pins and needles. These symptoms may be experienced in the same region as painful electric shock sensations.
  • Muscle Weakness:
    Because of the nerve irritation, signals from the brain may be interrupted causing muscle weakness. Nerve irritation can also be tested by examining reflexes.
  • Bowel or Bladder Problems:
    These symptoms are important because it may be a sign of cauda equina syndrome, a possible condition resulting from a herniated disc. This is a medical emergency, and your should see your doctor immediately if you have problems urinating, having bowel movements, or if you have numbness around your genitals.

All of these symptoms are due to the irritation of the nerve from the herniated disc. By interfering with the pathway by which signals are sent from your brain out to your extremities and back to the brain, all of these symptoms can be caused by a herniated disc pressing against the nerves.

What is Discectomy?

Discectomy is a surgical procedure in which the central portion of an intervertebral disc, the nucleus pulposus, which is causing pain by stressing the spinal cord or radiating nerves, is removed. Advances in options have produced effective alternatives to traditional discectomy procedures (i.e. Microdiscectomy, Endoscopic Discectomy and Laser Discectomy). A laminectomy is often involved to permit access to the intervertebral disc in a traditional Discectomy. The Intervertebral disc is the structure between the vertebrae (bones of the spine), which act as both a spacer and a shock absorber. The disc is composed of two parts: a soft gel-like middle (nucleus pulposus) surrounded by a tougher fibrous wall (annulus fibrosus).

Why Discectomy is done?

When an intervertebral disc ruptures in the lumbar spine, it puts pressure on one or more nerve roots (often called nerve root compression). This causes pain and other symptoms in the neck, arms and even legs. In this operation, the surgeon reaches the lumbar spine through a small incision in the low back. After the muscles of the spine are spread, the nerve root is exposed by removing part of the bony covering (lamina), which covers the nerve root. This is called a Discectomy. If necessary a portion of the intervertebral disc is then removed taking the pressure off of the nerve root.

How is a Discectomy performed?

A Discectomy is performed under general anesthesia. The procedure takes about an hour, depending on the extent of the disc herniation, the size of the patient and other factors. In order to remove the fragment of herniated disc, your surgeon will make an incision over the center of your back. The incision is usually about 3 centimeters in length. Using special instruments, your surgeon removes a small amount of bone and ligament from the back of the spine. This part of the procedure is called a laminotomy. Once the disc herniation is found, the herniated disc fragment is removed. Depending on the appearance and the condition of the remaining disc, more disc fragments may be removed in hopes of avoiding another fragment of disc from herniating in the future. Once the disc has been cleaned out from the area around the nerves, the incision is closed and a bandage is applied.

Recovery from Discectomy surgery:

Patients often awake from Discectomy surgery with complete resolution of their leg pain; however, it is not unusual for these symptoms to take several weeks to slowly dissipate. Pain around the incision is common, but usually well controlled with oral pain medications. Patients often spend one night in the hospital, but are usually then discharged the following day. A lumbar corset brace may help with some symptoms of pain, but is not necessary in all cases. Gentle activities are encouraged after discectomy surgery, such as sitting upright and walking. Patients must avoid lifting heavy objects, and should try not to bend or twist the back excessively. Patients should avoid strenuous activity or exercise until cleared by their doctor.

L5 Herniated Disc—Causes and Treatment

Our spine has 23 discs and each segment of the spine has a certain number of discs. They are identified by the name of the segment and the order of their placement.

For example, the disc C5 C6 means it is situated between the 5th and 6th vertebrae in the cervical region of the spine. In the same way, the lower or the lumber region of the spine also has discs.

 It has been noted that in most cases the disc herniation takes place in the lumber   spine between L4-L5 or L5-S1 vertebrae. Here, the prefix L stands for lumber nerve and S for sacrel nerve.

A disc herniation in the lumber region may impinge L5 nerve which may result in acute pain in the buttock, ankle and the big toe. There may be numbness on the top of the foot. Similarly, impingement of the S1 nerve may lead to numbness in the sole or the upper region of the foot.

Causes of L5 herniated disc

Usually the patient has a history of herniated disc, which is caused by  lifting heavy objects while bending or twisting at the lower back rather than at the knees. It may also occur due to accident, fall or a sudden violent motion in the spine.

 Treatment

L5 herniated disc can be treated in two ways, conservative and surgical.
The conservative treatment involves reducing pressure on the nerve root by doing stretching and strengthening exercises. They will make the muscles more flexible and soft.

Medication

The patient may take anti-inflammatory medicines, pain killers and relaxants. Cortisol injections may also help. It must be noted that this medication will bring only temporary relief. Moreover, an excessive use of such medicines may also cause side-effects.

Surgery

Surgery option should be taken as a last resort when all other therapies fail. The most common form of surgery for L5 herniated disc is known as micro-lumbar discectomy. It involves making a small incision. The patient is generally discharged the same day of surgery.
